The Faith Formation registration form is for all families who wish to enroll a child in our Faith Formation classes for grades 1-8. Any child who is not enrolled in a Catholic school (like ASA), should attend our Faith Formation classes.
If your child is preparing for First Communion or Confirmation (including ASA students), we will need two forms: this Faith Formation registration form and the appropriate Sacrament (First Communion or Confirmation) registration form also found on this page.

The First Holy Communion registration form is for all children who are planning to make their First Communion in the coming year. This includes children in public school, homeschooled and those enrolled in a Catholic school.
Sacrament prep is the responsibility of the parish, not the school. Therefore, St. Jude needs this form completed by all families (including ASA families) who have children preparing for First Communion.

The Confirmation Candidate registration form is for all children who are preparing to be Confirmed in the coming year. This includes children in public school, homeschooled and those enrolled in a Catholic school.

The OCIA Participant Information form is for all individuals aged 15+ who are preparing to come into full communion with the Catholic Church including those who were baptized Catholic, but have not received First Communion and/or Confirmation. OCIA participants do not need to fill out the Sacrament forms above. Only this form is needed.
The combined Diocese of Grand Rapids and St. Jude Media Release is for all individuals who participate in any aspect of our parish life. We value the privacy of all participants while celebrating the life of our parish community. Throughout the year, photographs, video recordings, and audio recordings may be taken during Masses, religious education, Vacation Bible School, youth ministry, parish events, service projects, and other parish activities.
